創建於一千五百年前的東晉末年，有普渡寺、杯渡寺、青雲觀等多個名稱，據古籍《高僧傳》所載，杯渡禪師喜歡乘坐一隻大木杯雲遊四海。青山禪院建築工程至1930年才告完成，內有杯渡禪師像。
This monastery features a statue of the Indian monk, Pui To. According to legend, Pui To liked to ride in a wooden cup to travel the world over.
